The form is designed to keep a very brief record of an intervention that has occurred between a First Responder and a member of the OneTRI community.
The purpose of this record is to evaluate the usage of the First Responder Network and ensure action is taken where required. Data collected with this form is confidential and will be reported back to the RAW Team/Shared Leadership Committee only.
